Frecuencia de cáncer y características clínicas asociadas en pacientes con Lupus Eritematoso Sistémico atendidos en el Servicio de Reumatología del Hospital Central de IPS de enero a diciembre del 2023

Introducción: La relación entre enfermedades autoinmunes sistémicas y el cáncer ha sido objeto de creciente interés. En el caso del lupus eritematoso sistémico, estudios previos han evidenciado un aumento en la incidencia de ciertos tipos de cáncer, posiblemente vinculados al daño crónico, la disfunción inmunológica y los tratamientos inmunosupresores. Sin embargo, la frecuencia y los factores vinculados al cáncer en pacientes con Lupus Eritematoso Sistémico en Latinoamérica siguen siendo poco explorados. Objetivo: Determinar la frecuencia de cáncer y analizar los factores asociados en pacientes con Lupus Eritematoso Sistémico atendidos en el Hospital Central del Instituto de Previsión Social. Metodología: Estudio observacional analítico que incluyó pacientes con diagnóstico de Lupus Eritematoso Sistémico atendidos entre enero y diciembre de 2023. Se recolectaron variables demográficas, clínicas y terapéuticas. La asociación con antecedente de cáncer se evaluó mediante pruebas de chi-cuadrado y U de Mann-Whitney. Resultados: Se incluyeron 193 pacientes, de los cuales el 5.18% presentó antecedente de cáncer. La variable significativamente asociada con el cáncer fue el índice de daño acumulado (p=0.007). No se encontraron asociaciones significativas con la edad, sexo, actividad de la enfermedad, comorbilidades, terapias inmunosupresoras, tabaco, alcohol ni autoanticuerpos. Conclusión: En esta cohorte, el antecedente de cáncer en pacientes con LES se asoció con un mayor daño acumulado medido por el SLICC, resaltando la importancia del control temprano de la enfermedad para reducir complicaciones a largo plazo.
