Resumen

Introducción: La anquiloglosia, caracterizada por la restricción del movimiento lingual debido a un frenillo con inserción anómala o tenso, es una de las causas orales frecuentemente subdiagnosticadas asociadas al fracaso de la lactancia materna exclusiva (LME). Pese a que su prevalencia reportada oscila entre el 4,2% y el 15%, persiste la ausencia de un consenso diagnóstico universal, lo que subraya la necesidad de abordar esta condición desde una perspectiva funcional integral, más allá de la mera evaluación anatómica. Objetivo: Sintetizar la evidencia científica actual sobre la anquiloglosia y su impacto en la lactancia materna, analizando definiciones, clasificaciones, herramientas de evaluación anatómico-funcional (tales como Hazelbaker/ATLFF, BTAT, TABBY y el Protocolo de Martinelli) y de lactancia (LATCH), así como la evidencia disponible respecto a la eficacia de la frenotomía. Conclusión: La evaluación funcional integral resulta indispensable, dado que la severidad anatómica no siempre se correlaciona con la magnitud de la disfunción. Se concluye que la implementación de herramientas de tamizaje estandarizadas, accesibles para los profesionales de atención primaria y odontopediatras, representa una estrategia fundamental para optimizar la detección temprana, evitar intervenciones innecesarias y fortalecer la lactancia materna exclusiva como pilar de la salud pública materno-infantil.
