Abstract

Introduction: Ankyloglossia, characterized by restricted tongue mobility due to a tight or abnormally inserted lingual frenulum, is one of the most frequently underdiagnosed oral causes associated with breastfeeding failure. Although its reported prevalence ranges between 4.2% and 15%, there is still no universal diagnostic consensus, which underscores the need to address this condition from a comprehensive functional perspective, beyond mere anatomical assessment. Objective: To synthesize current scientific evidence on ankyloglossia and its impact on breastfeeding by analyzing definitions, classifications, anatomical-functional assessment tools (such as Hazelbaker/ATLFF, BTAT, TABBY, and the Martinelli Protocol) and breastfeeding assessment tools (LATCH), as well as the available evidence regarding the efficacy of frenotomy. Conclusion: Comprehensive functional assessment is essential, as anatomical severity does not always correlate with the magnitude of dysfunction. The implementation of standardized screening tools, accessible to primary care professionals and pediatric dentists, represents a fundamental strategy to optimize early detection, avoid unnecessary interventions, and strengthen exclusive breastfeeding as a pillar of maternal-infant public health.
