EX VIVO EFFICACY OF NEOLIX AND PROTAPER NEXT ROTARY SYSTEMS IN THE RETREATMENT OF ROOT CANALS OF MAXILLARY MOLARS

Abstract

Objective: To compare ex vivo the efficacy of the Neolix-NeoNiti and ProTaper Next rotary systems for filling-material removal and reshaping of root canals in the palatal roots of maxillary first molars during endodontic retreatment, including as a baseline reference the quality of the initial root canal filling prior to retreatment. Materials and Methods: Ex vivo comparative experimental study. From a bank of 80 maxillary molars, 40 apically mature teeth with a complete palatal root were selected, endodontically treated for the first time, and randomly allocated to two groups of 20: Group 1, retreated with Neolix-NeoNiti (France), and Group 2, with ProTaper Next (Dentsply Maillefer, Switzerland). In the cervical, middle and apical thirds of the palatal root, residual filling material, canal transportation/deviation and instrument fracture were assessed using cone-beam/computed tomography (CT), digital radiovisiography (RVG) and operating microscopy (Newton, 16-125x). Chi-square, Fisher's exact test and factorial ANOVA were applied (p<0.05). Results: CT showed no significant between-group differences in any third (p>0.05), but significant differences across thirds, with more residual material in the apical third (22.5%) than in the cervical (7.5%) and middle (2.5%) thirds (p=0.011). RVG showed more residue with ProTaper Next in the middle third (20.0% vs. 0.0% with Neolix; p=0.035) and greater apical transportation with ProTaper Next (25.0% vs. 10.0% with Neolix; overall 11.7% vs. 3.3%). Instrument fracture was infrequent and did not differ significantly between groups (Neolix 6.7%, ProTaper Next 3.3%; overall 5.0%; p>0.05).
